Which forms do I need to complete?
If you are...
...New to the Troop, & need to provide a health form before attending a Troop-specific trip, you only need to complete parts A, B1, and B2. These can be completed at home, and do not typically require a physicians visit.
...Attending Summer Camp or a High Adventure trip, you will also need a completed part C, and a print out of your most recent immunizations. These must be provided, and signed by, a licensed medical practitioner.
How do I submit completed forms?
Please print two physical copies, and include a copy of your health insurance card.
Then, deliver them to your Scoutmaster prior to attending your first Scout event.
Scouting does not allow us to keep digital copies of health forms, so it's important that physical copies are provided.
We need two copies because one copy will accompany the troop on events that you attend, while the other will serve as a safe backup.
I already submitted this last year. Do I need to summit a new one?
BSA health forms are only valid for 1 year.
However, the good news is that Scouts attending both Yawgoog and High Adventure camps in the same summer only need to submit these forms once for that year.
